Each service is funded through NJ DDD and Medicaid waivers, delivered by trained Direct Support Professionals, and shaped around the goals on a person's Individualized Service Plan.
Community-Based Supports
Support that helps individuals actively engage in their communities and build meaningful daily routines — skill-building in community settings, social interaction support, and participation in everyday activities.

Individual Supports
One-to-one assistance to help individuals live more independently and achieve personal goals — daily living support (hygiene, meals, routines), life skills development, and goal-based support.

Respite
Short-term care providing relief for caregivers while maintaining continuity of care — temporary supervision, routine support, and safe engagement.

Safe and reliable transportation for access to daily activities — travel to work, programs, appointments, and community participation.
