DigniPath takes the privacy of the individuals, families, and partners we work with seriously. This page explains what we collect through this website, how we use it, and the choices you have.
What this policy covers
This policy applies to information collected through dignipath.com and through email, phone, or form submissions made to DigniPath. It does not cover protected health information (PHI) handled inside our service delivery — that information is governed by HIPAA and our participant service agreements.
Information we collect
- What you send us. Name, email, phone, county, and any details you include in our contact, family inquiry, referral, or careers forms.
- Basic site analytics.Standard request logs (pages viewed, referring page, approximate location from IP, device and browser type) used to keep the site working and to improve content.
- Cookies. We use only the cookies required for the site to function. We do not use advertising or cross-site tracking cookies.
Please do not include Social Security numbers, Medicaid IDs, diagnoses, or other protected health information in website forms or email. We will follow up through a secure channel for those details.
How we use information
- To respond to your inquiry, referral, or job application.
- To coordinate intake with support coordinators and families.
- To operate, secure, and improve this website.
- To meet legal, regulatory, and Medicaid waiver documentation requirements.
We do not sell or rent personal information to anyone.
Who we share it with
We share information only with: (1) DigniPath staff who need it to respond to you, (2) the support coordination agency or state program your inquiry relates to when that is the purpose of your contact, and (3) service providers that host our website or email — under contracts that require them to protect your information. We will also disclose information when required by law.
How long we keep it
Inquiry and referral information is kept while it is useful to serve you and for as long as required by NJ DDD documentation and tax/employment recordkeeping rules. Job application materials are retained for up to one year unless you ask us to delete them sooner.
How we protect it
The site is served over HTTPS. Access to inquiry and referral data is limited to authorized staff. Records that contain PHI are stored in HIPAA-aligned systems used for service delivery — separate from public website data.
Your choices
- Ask what information we have about you.
- Ask us to correct or delete information you sent through the site.
- Opt out of any non-essential communications at any time.
