We prioritize consistent staff assignments whenever possible. Changes are communicated in advance, and introductions are managed carefully to ensure continuity, comfort, and safety.
Support Coordinators are never the last to know. Attendance issues, behavioral changes, or family concerns are communicated promptly so adjustments can be made collaboratively, in alignment with the ISP and NJ DDD/Medicaid requirements.
Some individuals require additional structure, gradual engagement, or specialized strategies. DigniPath works with families and care teams as well as collaborates to develop individualized, person-centered plans that ensure safety, measurable outcomes, and consistent progress.
