If you're new to NJ DDD services — or just new to us — this page walks through what DigniPath offers, how to start, and what to expect along the way.

Getting started

DigniPath provides services to individuals who are enrolled with the New Jersey Division of Developmental Disabilities (DDD) and receive Medicaid-funded supports.

Our five core services — Community-Based Supports, Individual Supports, Respite, Transportation, and Supported Employment — are all funded through NJ DDD and Medicaid waivers, with no direct cost to families who qualify.

Our person-centered intake process

  1. 1. Referral received. Call, email, or have your Support Coordinator send a referral.
  2. 2. Eligibility verified. We confirm DDD enrollment and Medicaid-funded services.
  3. 3. Intake meeting. We meet to understand goals, preferences, and routines.
  4. 4. Services matched. We pair the individual with the right Direct Support Professional.
  5. 5. Services begin. Support starts with trained staff, aligned with the ISP.

What to bring to a first conversation

  • Your loved one's name, age, and county of residence
  • The name of your Support Coordinator (if you have one)
  • A copy of the current Individualized Service Plan (ISP), if available
  • The services you're interested in — or your questions if you're unsure
  • Schedule constraints (school, work, medical appointments, etc.)

Frequently asked questions

Who pays for these services?

Our services are funded through NJ DDD waivers and Medicaid. For families who qualify, there is no direct out-of-pocket cost. If you're unsure about eligibility, your Support Coordinator can help — or we'll walk you through it.

Do we need a Support Coordinator first?

In most cases, yes — a Support Coordinator (assigned through NJ DDD) authorizes the services we provide. If you don't have one yet, contact NJ DDD to request one, and we're happy to point you in the right direction.

How long does it take to start services?

Most participants begin services within 10–14 days of referral. Within 48 hours of hearing from you, we respond, screen, and review documentation; days 3–7 we meet in person and review clinical and safety needs; days 7–10 we share an acceptance decision and complete intake; days 10–14 we match a DSP and finish training so services begin with purpose at the end of week two.

Can we choose the staff member who comes to our home?

We match DSPs on fit, not just availability — looking at schedule, language, personality, and the routines that matter to your loved one. You meet the proposed DSP before services start, and we keep the same person assigned wherever possible so the relationship stays consistent.

What if the match isn't working?

Tell us early. We'll listen, adjust, and re-match if needed. We'd rather make a change the first week than have anyone settle for support that isn't quite right.

Which counties do you serve?

Based in Phillipsburg, we serve Warren as our primary base, with core coverage in Hunterdon, Morris, Somerset, Middlesex, Union, and Essex. We are not currently onboarding in South Jersey or Shore regions — if we can't meet your need, we'll say so up front and help point you to a trusted partner.
