Community Based Supports

Help individuals build skills and take part in the everyday life of their community — at their pace, on their terms.

What it is

Community Based Supports help an individual learn, practice, and apply the skills needed to participate in their community. That can mean using public transit, volunteering at a neighborhood organization, taking a class, building friendships at a recreation center, or simply running errands with confidence.

Who it's for

  • Adults receiving services through NJ DDD with Community Based Supports authorized on their plan.
  • Individuals working toward goals around independence, community participation, or social connection.
  • Families who want their loved one engaged beyond the home, with the right level of support.

What to expect

A DSP is matched to the individual based on interests, schedule, and goals. Sessions happen in the community — not in an office — and progress is documented in clear, plain-language notes shared with the Support Coordinator.

How it's funded

Community Based Supports are covered under the NJ DDD Supports Program and Community Care Program (Medicaid waiver). The hours and budget are set in the individual's Individualized Service Plan (ISP) and authorized by the Support Coordinator.

How to start

Support Coordinators can submit a referral directly. Families and individuals can reach out to our intake line — we'll walk through next steps together.
