Individual Supports

One-to-one help shaped around a person's daily routines, household needs, and personal goals.

What it is

Individual Supports cover the practical help an adult needs to live as independently as possible — meal prep, household tasks, personal care routines, scheduling, and follow-through on the goals in their Individualized Service Plan.

Who it's for

  • Adults living at home, in their own apartment, or with a family member.
  • Individuals who benefit from a consistent person helping them through the day.
  • Participants whose ISP authorizes Individual Supports under NJ DDD.

What to expect

A DSP is matched based on schedule, language, and personality fit. Visits happen on a steady cadence, and notes are shared with the Support Coordinator and family on the rhythm everyone agrees to.

How it's funded

Individual Supports are authorized through the NJ DDD Supports Program or Community Care Program (Medicaid waiver). The number of hours is set on the ISP.
