How to apply for NJ DDD, one step at a time

Applying to the NJ Division of Developmental Disabilities (DDD) is a paperwork journey. Here's the whole path in plain English, in the order it happens.

Before you start: who qualifies

NJ DDD serves people age 21 and older with a developmental disability that started before age 22 and substantially limits daily life. Children under 21 stay on school-based services (IEP) and Special Child Health Services until they age out.

Age 18 is the paperwork start. You can (and should) apply as early as age 18, even though services don't begin until age 21. Early paperwork means no gap in supports.

The 6 steps

Step 1: Get NJ Medicaid first

DDD services are funded through Medicaid. If your loved one isn't already on Medicaid, apply through NJ FamilyCare or your County Board of Social Services. See our Medicaid guide.

Step 2: Request the DDD Intake Packet

Call the DDD Community Services office for your region or download the Application for DDD Eligibility from NJ.gov. Ask specifically for the "Intake Packet."

Step 3: Gather documents

Birth certificate, Social Security card, Medicaid card, proof of NJ residency, psychological or IQ testing (dated before age 22), IEPs, medical records, and any diagnosis letters from doctors.

Step 4: Submit and wait for the eligibility decision

DDD reviews your packet. You'll get a written decision: Eligible, Not Eligible, or Request for More Information. This can take 60 to 90 days.

Step 5: Complete the NJCAT assessment

Once eligible, DDD schedules the New Jersey Comprehensive Assessment Tool (NJCAT). A trained assessor interviews you and the individual to score support needs. This determines your annual budget tier.

Step 6: Choose a Support Coordination Agency (SCA)

You'll receive a list of SCAs in your county. Pick one, sign the enrollment form, and your Support Coordinator will guide you through building the first Individual Service Plan (ISP).

If you get denied

A denial is not the end. You have 20 days from the date on the letter to request a Fair Hearing. Many families win on appeal by adding evidence of pre-age-22 onset (school records, early evaluations). See our Appeals guide.
