About Us

Who We Are

DigniPath is a New Jersey DDD and Medicaid-approved support provider serving individuals with intellectual and developmental disabilities across the state. We provide structured, person-centered supports in homes and communities that align with each individual’s approved ISP.

All supports are delivered predictably, documented accurately, and coordinated with care teams.

What Makes Us Stand Out?

We combine structured, person-centered support with predictable operations to reduce stress for families and care teams while promoting positive outcomes for individuals.

Key Differentiators:

  • Predictable Staffing & Continuity
Whenever possible, the same staff are assigned to each individual. Planned transitions are communicated in advance to maintain stability.

  • Proactive Communication
Families and care teams are informed early about concerns, attendance, or changes—preventing surprises and ensuring collaborative solutions.

  • ISP-Aligned Services
All supports are delivered according to each individual’s approved ISP and documented accurately to track progress and outcomes.

  • Experienced, Trained Staff
Our team receives ongoing training in DDD service standards, person-centered practices, and behavioral support strategies.

  • Collaboration & Support for Complex Needs
We work closely with families and care teams, including for challenging placements, to develop manageable support plans that prioritize safety, stability, and measurable progress.

These operational standards ensure predictable, low-risk support that families and care teams can rely on.
